Evidence of Insurability (EOI), also known as “Proof of Good Health”, is a simplified record of a person's medical history. Insurance companies ask for an EOI to verify whether the person applying for insurance meets their requirements for reasonably good health.
During the annual Open Enrollment period, anyone electing to add any of the following products for themself or their partner will be required to submit an EOI:
- Supplemental Life insurance
- Long Term Disability
- an increase in coverage amounts for previously enrolled life insurance products
New Hires are also asked to submit an EOI if they elect insurance coverage amounts that exceed the guaranteed issue amount.
Please Note that if an EOI is required but not submitted, insurance elections or changes will not be approved.
